Twisted weald, prism of light

after summoning dreadhorn and killing 2 times, on the third time dropping keys we entered dread horn area and NO new dread horn had spawned.
This exact same thing happened last week when chaining shimmering effusion. only it happened on the second time summoning him. we entered shimmering and there was no peerless.
both resulting in losing a set of peerless keys.

Could someone have stealthed in following you, then stayed inactive?

There is a spawn delay. I have gotten the message at Travesty that I must wait before summoning again. There could be an issue with the altar, where it isn't prevent players from re-entering and/or dropping keys before the previous instance has expired. Could try to wait until you get kicked before trying again.
